IMPLANTS WITH IMMEDIATE LOADING

The growing demand among patients to achieve restoration as soon as possible has led to shorter classic osseointegration periods.

The great advantage of the Immediate Loading technique is that the implants can be inserted on the same day and a temporary fixed prosthesis can be placed on them.

The immediate loading technique can be optimally achieved by a surgical-prosthetic team.

Immediate functional loading can be used for the replacement of single teeth, for arches that have lost some teeth, as well as for arches that have lost all teeth. This technique allows implants to be placed in the appropriate position for each patient, followed by the adaptation of the temporary prosthesis before closing the flaps. Patient without systemic alterations that could alter healing (periodontitis, tobacco, alcohol, diabetes), etc.

• Ideal quality and quantity of bone.
• Absence of micro movements (axial axis).
• Ferrule effect between implants.
• Correct occlusal factors.
• Adequate morphology and surface area of the implant.
• Surgical technique.
• Primary stability of the implant.

The stability of the implant after insertion is the most important success factor for osseointegration. The primary stability of implants is initially determined by bone density, trabecular bone structure, the surgical technique, the number, self-tapping design and length of the implants used and their distribution around the dental arch.

Using temporary prostheses during the healing period gives us the opportunity to assess aesthetics, speech, etc. The functional load during the osseointegration period is 2/3 months for the jaw, and 3 to 6 months for the upper jaw.

Not all patients and not all anatomical bases are indicated for teeth with immediate loading. It is highly recommended that patients be put on a soft diet during the 3 month healing period, to limit functional forces while osseointegration takes place. Because this is not easy to do, patient compliance is a major factor in achieving success.

It is widely recognized that occlusal forces are greater in the rear sector than at the front. Loading a single-shaped molar will always be a contraindication (short implant, rotational forces and low quality jaw bone, etc.)

Immediately loading an entire arch will always be the safest option, if all other factors are correct.

In terms of aesthetics we will perform IMMEDIATE AESTHETICS, which is the same thing, but we avoid contact with the new piece during any chewing movements, as it is during chewing that no movements of the implant are along its axial axis. Very common when replacing fractured teeth, for example due to trauma to the back teeth.

When we can combine immediate implants and immediate loading, it means that a patient who comes in for an appointment with teeth in poor condition can leave after the work with a healthy peridontium, with implants fitted and a tooth or teeth fixed onto them, thereby immediately recovering their functions.
